We work in the following ways to support people to shift from treatment to prevention:
We equip individuals with the skills and knowledge to be in control and manage their own health.
We carry out clinical and non-clinical health checks within each workshop delivery. This helps participants understand their health profile so they can make informed lifestyle changes
Promoting regular NHS Health Checks is one of our major workshop programme features, helping people understand the importance of prevention and early identification of health issues.
Encourage and support people to use digital tools such as downloading apps, using social media, email, WhatsApp chat, etc in communicating with us and other health and social care providers.
Link people to local, community-based services that address their problems, such as diet, isolation support groups, and exercise classes, to build resilience and maintain a healthy lifestyle.
Skip Diabetes is a mobile workshop that can be delivered in any comfortable environment, be it clinical or non-clinical, which promotes discussions on blood glucose, blood pressure, healthy food choices, physical activity levels and their impacts, and types of fats and their effects.
How we support people from analogue-to-digital
We encourage and support people to use digital tools such as downloading apps, using social media, email, WhatsApp chat, etc in communicating with us and other health and social care providers.
Family, friends and relatives, where available, are used to support individuals’ transition from analogue to digital.
Vulnerable individuals who rely on landline telephones are regularly monitored, practically and emotionally supported as needed.
